postconf -n では smtpd_xxx_restrictions は smtpd_recipient_restrictions
しか見当たらないのに、拒否が >>> START Client host RESTRICTIONS <<< の中
で発生していて条件が permit_sasl_authenticated しか見当たらないのが不思
議ですね。

私には、

smtpd_delay_reject=no を指定してみて、どの段階で拒否がされているか確認する。

postconf | grep restriction を実行してみてデフォルト値を含めて設定を確認
する。

ぐらいしか思いつきません。
