sampleファイルの中に該当するものがあったことに気付かなかったのはお恥ずかしい限りです。
さて、main.cfに
allow_mail_to_commands = alias,forward,include
allow_mail_to_filess = alias,forward,include #将来のため
を付け加えることにより

*** $dir for hackers is created.

*** config.ph for hackers is created.

*** include for hackers is created.

*** empty members.ph for foo is created.

*** admins.ph for hackers is created.

Q and Aをpostfix-jpで作っておられるのでしたら、このようなQ and Aを作っ
てみました。このようなものはどちらに送ればよろしいのでしょうか？

MLドライバをインストールして書いてある通りにMLドライバを管理するユーザー
あてにメールを送ると
<mldriver@xxxxxxx>: mail to command is restricted
といった旨のエラーメッセージが返ってきます。

A.
多くのMLドライバでは:include:によりコマンドを実行しますが、
Postfixはdefaultでは :include: ファイルでの "|command" への配送は許可
しません。

main.cfに下記の行を加えて :include: ファイルでのcommand配送を許可して
ください。
allow_mail_to_commands = alias,forward,include
